Ventura County Office of Education (VCOE) has been with CSEBO for medical, dental and vision benefits since 1991. Lisa Cline and Theresa McGee are the current Board representatives for VCOE, with Misty Key and Gloria Ortega, representative of the Ventura County Federation of Paraeducators, serving as Alternates. VCOE 2018-19 Healthy Habits Incentive Program

Getting regular checkups with your Primary Care Physician (PCP) can help you stay well and catch problems early. It may even save your life.

For any employee who receives a qualified preventive care visit or age-appropriate screening from June 1, 2018 through May 31, 2019 with their healthcare provider will receive a $50 incentive. For those employees who participated in the 2017/18 program and received a $50 incentive, will receive another $50 incentive and an additional $25 for participating in the program two or more years in a row.

Do I need to fill out a form?
If you are currently enrolled in the Anthem PPO or Kaiser Permanente, you do not need to fill out a form. Just complete your preventive care or age-appropriate screening with your PCP and we will process automatically. If you are currently enrolled in the Anthem HMO or are non-benefitted (do not take benefits through VCOE), but are benefit-eligible, please fill out the Healthy Habits Incentive Claim Form and return it to CSEBO.

Anthem Blue Cross Diabetes Prevention Program

Anthem Blue Cross, in conjunction with Solera Health, are offering Anthem HMO and PPO members a free voluntary program to help you lose weight and significantly reduce your risk of developing diabetes. If eligible, the 16-week program gives members access to: small group support, a personal health coach, tools like a wireless scale or activity tracker, and weekly lessons from an array of national and local programs, like Weight Watchers, Retrofit and Jenny Craig.
